Open Question: What is wrong with this horse?

I don't own this horse or I would have already had the vet check her over. It blows my mind how some people can own animals without being able to afford proper care, I believe we owe it to them. My friend isn't barbaric but I'm gathering that she doesn't know a lot, and if I could pinpoint what was wrong with this horse she may heed my advice and go in debt for the poor animal. She keeps 5 horses in an outdoor pasture attached to several large run in sheds with bedding. The horses live outside 24/7 without blankets, but they have access to round bales in two different feeders. 4 of the horses she owns are bright, responsive, and seemingly in good health, although I've only seen them in pasture. Her 4 year old mare looks sickly, thin and depressed from any angle. I've been told that the horse was born on my friend's property and hadn't any health problems. Just last year, apparently at the beginning of the summer, she was sent to another person's farm so they could begin training her. Apparently my friend trusted the method these people used to train, but when she got back late fall, she was "quiet". She started losing a lot of weight and the first time I saw her in early december, she was at her worst. I saw her last week and she hasn't improved whatsoever. I was shown pictures of her before she left the farm, and she had a really nice full shape. Now she's ribby, her back is sunken in slightly, as with her neck. She looks depressed, and I've never seen a horse depressed. One of my first guesses was automatically strangles, which is a terrifying thought for her other horses. Is it possible that the disease has been laying dormant in her for awhile? How long does it take for the lymph nodes to swell and puss? The lymph nodes aren't swollen and none of the other horses are showing any symptoms. I believe one of her horses is quite old (looks so), but all of her horses seem healthy and perky except for the one. My second guess is a problem with her molars, but at 4? She could have been injured, but wouldn't her face be swollen if it was anything like that? There is no nasal discharge or drooling from what we can see. I do recall seeing her nibbling at the round bale, but not very enthusiastically. Anything else is beyond me. My best interest is for this poor creature who obviously deserves the attention of a professional. Any sound advice would be greatly appreciated, and likely followed. Although it isn't any of my business outright telling her what to do, my goal will be to persuade her to have the vet come take a look, and perhaps do some blood work.
